BANKRUPTCY CHECKLIST

[   ]       Date and Case Number of any previous Bankruptcy Discharge(s)

[   ]       Credit Report (annualcreditreport.com) – Experian is the easiest to read

[   ]       Certificate of Credit Counseling (I will sign you up for a class before you file)

[   ]       Real Estate Deed* & Property Tax Statement (* copy of 1st recorded page) & BPO, Market Analysis or Appraisal of Fair Market Value (FMV)

[   ]       Mortgage*, Land Contract*, Residential Lease (*copy of 1st recorded page)

[   ]       Loan Modification Agreement

[   ]       Vehicle Title(s) & Lien Termination Statement(s) & Kelly Blue Book FMV

[   ]       Vehicle Loan/Lease Agreements

[   ]       List of Personal Property showing FMV (See Personal Property Itemization List)

[   ]       Financial Account Statements – Bank, CU, Retirement, 401(k), Educational      Savings Accounts, Closed Accounts, Set Offs  (Past 6 months)

[   ]       Insurance Policies: Life (Cash Value); Auto; Home; Other

[   ]       Current IRS & Michigan Treasury Notices (Including Levies)

[   ]       Domestic Support Orders (Alimony & Child Support)

[   ]       All Bills (Current Statements) & Collection Agency Notices – List All Payments to Creditors within Last 3 Months

[   ]       Names & Current Addresses of Co Debtors

[   ]       Wage & Benefit Statements & Pay Stubs (Past 6 months)

[   ]       Statements for All Other Income – SSI, EBT, DHS, FIA, etc.

[   ]       Monthly Expenses (See Monthly Budget Itemization List)

[   ]       Signed & Filed Income Tax Returns (Past 2 Years)

[   ]       Law Suits, Judgments or Other Claims (for or against you including Personal  Injury, Insurance Loss & Divorce)

[    ]      Garnishments, Repossessions &  Foreclosures (including dates for each and the amount of each garnishment for the past 3 months)
